Improving access to GP Appointments, Community Phlebotomy clinics and Health Screening.
There are around 26 million patients in the UK with at least one chronic condition, placing a heavy funding strain on NHS services and local authorities.
It is imperative for the long term sustainability of the NHS that we continue to create wider access to care and disease screening within our local communities.
At Direct Access Healthcare we support the planning and development of primary care services, we focus on improving access to screening programmes, particularly the NHS Health Check.
There are some quick wins that can deliver improvements at scale, such as:
- Increase uptake of the NHS Health Check service
- Provide clear and effective care signposting for patients
- Establishing Healthy Living Programmes
- Introduce Nurse lead mentoring
- Encouraging patients to utilise digital health initiatives
- Measuring improved patient outcomes
- Improved coordination with Social Care provision
By helping you to increase the availability of disease screening and prevention services we can fundamentally reduce the burden on the secondary care sector allowing it to focus on providing world class emergency and critical care.
